What to Expect:
- Please bring the intake form if you are able to download it and fill it out in advance.
- Bring Prescription and Insurance Cards.
- We are currently enrolled in Medicare, Medicaid, Blue Cross, Aetna, Centene (Ambetter), Meridian, Molina, Blue Cross Community, Well Care and VGM/Homelink, for both Orthotics and Prosthetics. We also take Workers Comp.
- In addition, we have the ability to accept for Prosthetics :

- If we are not in network, we often can utilize your out of network benefits.
- We are in the process of obtaining other network direct contracts for both Orthotics and Prosthetics.
- We also accept CareCredit.
Due to the Covid-19 pandemic:
- Masks are required at all times. Only one caregiver may accompany each patient to an office visit. Bring your mask to your appointment.
- When you arrive, temperature and oxygen level screenings are performed at entry for everyone's safety.
- If you are sick and have respiratory symptoms, such as fever, cough, and shortness of breath, stay home and please call to reschedule your appointment. Covid-19 symptoms include:
- Fever
- Cough
- Shortness of breath or difficulty breathing
- Chills
- Repeated shaking with chills
- Muscle pain
- Headache
- Sore throat
- New loss of taste or smell
